Achievements
The Transition project started the process of improving the quality of transition care for children and young people with cancer across the South Thames Operational Delivery Networks by:
Developing and promoting a shared understanding of best practice for cancer transition.
Providing resources and information on transition for patients, their families and healthcare professionals, find the Transition Hub here.
Developing a community of practice to share experiences, ideas and resources and a community of support for professionals wanting to benchmark and improve transition.
How were patients and others involved?
45 young people and parents were asked if they would like to get involved.
Everyone contacted voiced an interest in the project, with many choosing to share their own experiences. This greatly contributed to understanding of the current practice, and generated enthusiasm for regarding potential opportunities for change.
13 individuals (7 young people and 6 parents) remained involved for the duration of the project across all areas of work.
